The effect of music to improve sleep quality in depression related insomnia

Introduction
Insomnia in depression is common and difficult to resolve. Music is commonly used as a sleep aid, and clinical trials pointing to positive effects of music as a sleep aid are increasing adding to the evidence base. There is little knowledge on the effectiveness of music for depression related insomnia.
ObjectivesA recent RCT study conducted in psychiatry at Aalborg University Hospital examined effects of a music intervention for insomnia in depression. The intervention group listened to music at bedtime for four weeks, controls were offered music intervention post-test. Primary outcome measure was Pittsburgh Sleep Quality Index (PSQI). Secondary outcomes included Actigraphy, The Hamilton depression Rating Scale (HAMD-17) and World Health Organisation well-being questionnaires (WHO-5, WHOQOL-BREF).
MethodsA two-armed randomized controlled trial (n=112) and a qualitative interview study (n=4)
ResultsThe RCT study showed signficant improvements for the music intervention group in sleep quality and quality of life at four weeks according to global PSQI scores (effect size= -2.1, 95%CI -3.3; -0.9) and WHO-5 scores (effect size 8.4, 95%CI 2.7; 14.0). Actigraphy measures showed no changes and changes in depression symptoms (HAMD-17) were not detected.
The interview study unfolded examples of the influences of music on sleep and relaxation. Music distracted, affected mood and arousal positively and supported formation of sleep habits.
Results from the trial are discussed and merged with findings from the interview study. The results from the trial suggested moderate effects of music listening for the population while findings from the interview study showed examples of individual and highly varying outcomes.
ConclusionsMusic is suggested as a low-cost, side-effect free and safe intervention in supplement to existing treatments improving sleep in depression.

Causes of pre-slaughter mortality in Danish slaughter pigs with special emphasis on transport

Annual statistics for transport and lairage mortality were used to investigate factors leading to pre-slaughter mortality in Danish pigs. A subset of material from 2002, amounting to 17.8 million pigs, was used for a more detailed study of the effect of producer, haulier, abattoir and transport distance (< 100, 100–200 and > 200 km) on transport mortality. Total mortality was reduced eight-fold during the period that the halothane gene was being removed from the pig population, from 0.12% in the early 1980s to 0.016% in 2002. Overall, transport mortality increased with higher temperature and lower relative humidity/wind speed but a combination of temperature and humidity that fell into the danger zone, as defined by the Livestock Weather Safety Index, almost doubled transport mortality from a level of about 0.016 to 0.031%. Multiple deaths on the same transport were also more frequent during the hotter months of the year. Transport mortality increased with increasing transport distance, especially during warmer weather. Producers, hauliers and abattoirs had widely varying transport mortality. Eighty-nine percent of producers, 11% of hauliers and 86% of farmer transports had no mortality at all. Producers and farmer transports supplying less than 1,000 pigs had a higher transport mortality than those supplying more pigs, whereas it was independent of supply for hauliers. There were many confounding factors in this work, as producers, hauliers and abattoirs are linked and trends over the years can be affected not only by changes in the genetic makeup of the pig population but also by improvements in handling. Nevertheless, the study shows that internal environment within the vehicle and transport pattern, including the time the vehicle is stationary, are important factors for mortality and that particular efforts should be made if weather forecasts predict dangerous combinations of temperature and humidity. It is suggested that efforts to further reduce transport mortality in Danish pigs should concentrate especially on these factors and include a routine follow-up of all multiple deaths to pinpoint specific factors leading to mortality.

Aims:
To compare Lithium prescribing practices in a Psychiatry of Old Age (POA) Service in the North-West ofIreland among adults aged 65 years and over with best practice guidelines.
Methods:Review of the literature informed development of audit standards for Lithium prescribing. These included National Institute for Clinical Excellent (NICE) 2014 guidelines, The British National Formulary(2019) and Maudsley Prescribing Guidelines (2018). Data was collected retrospectively, using an audit-specific data collection tool, from clinical files of POA team caseload, aged 65 years or more and prescribed Lithium over the past year.
Results:At the time of audit in February 2020, 18 patients were prescribed lithium, 67% female, average age 74.6 years. Of those prescribed Lithium; 50% (n=9) had depression, 44% (n=8) had bipolar affective disorder (BPAD) and 6% (n=1) schizoaffective disorder.
78% (n= 14) of patients met the NICE standard of 3-monthly lithium level. Lithium levels were checkedon average 4.5 times in past year, average lithium level was 0.61mmol/L across the group and 39% (n=7) had lithium level within recommended therapeutic range (0.6-0.8mmol/L).
83% of patients (n=15) met the NICE standards of 3 monthly renal tests. Taking into consideration mostrecent blood test results, 100% (n=18) had abnormal renal function.
Half (n=9) were initiated on lithium by POA service and of these, 56% (n=5) had documented renal impairment prior to initiation. Of patients on long term lithium at time of referral (n=9), almost half (n=4) had a documented history of lithium toxicity.
Conclusions:The results of this audit highlight room for improvement in lithium monitoring of older adults attending POA service. Furthermore, all patients prescribed lithium had impaired renal function. This is an important finding given the associations between those admitted to hospital with COVID-19 and co- morbid kidney disease and increased risk of inpatient death.
Our findings highlight the need for three monthly renal function monitoring in elderly prescribed lithiumgiven the additive adverse effects of increasing age and lithium on the kidney. Close working with specialised renal services to provide timely advice on renal management for those with renal impairment prescribed lithium is important to minimise adverse patient outcomes.

INTRODUCTION:
Lithium has a narrow therapeutic index with a risk of toxicity and potential to increase morbidity and mortality, particularly in the elderly with co-morbid illness and polypharmacy. Lithium therapy and monitoring of lithium levels require precision and several guidelines have been issued including recommendations for strict control of lithium levels in the elderly population.
RESEARCH OBJECTIVE:We evaluated the effect of implementation of a multifaceted management programme on the compliance with international practice standards for lithium monitoring in patients under the care of Psychiatry of Old Age (POA), Sligo Leitrim Mental Health Services in the North West of Ireland.
METHODS:Results from a prior audit performed in February 2020 involving a cohort of 18 patients prescribed lithium under the care of POA were analysed and compared to accepted standards. The guideline used as the benchmark for compliance was the National Institute for Clinical Excellence (NICE) guidelines on the use and monitoring of lithium therapy, as published in 2014. Several deficits were found and therefore a designated Lithium Management Programme was established. A subsequent audit, performed using a simplified audit tool incorporating the NICE guidelines, delivered results which were directly compared to the initial findings and analysed to evaluate the effect of the implemented management programme.
PRELIMINARY RESULTS OF THE ONGOING STUDY:In comparison with findings from 2020, there had been a significant improvement in most facets of lithium management and compliance to practice standards. Of particular note was the improvement of biochemical monitoring, side effect screening, polypharmacy surveillance, patient education and interdisciplinary communication.
CONCLUSION:The launch of a dedicated Lithium Management Programme with specific features aimed at identifying and addressing poor compliance with monitoring guidelines has led to improved adherence to accepted international practice standards. Our model provides a dynamic, multi-layered system which paves the way for better patient outcomes, timely access to care and furthering education for patients and staff members.

Background
Alzheimer's disease (AD), the most common disease causing dementia, is linked to increased mortality. However, the effect of antipsychotic use on specific causes of mortality has not yet been investigated thoroughly.
MethodsUtilizing the Danish nationwide registers, we defined a cohort of patients diagnosed with AD. Utilizing separate Cox regressions for specific causes of mortality, we investigated the effects of cumulative antipsychotic dosage after diagnosis and current antipsychotic exposure in the time period 2000–2011.
ResultsIn total, 45,894 patients were followed for 3,803,996 person-years. A total of 6129 cardiovascular related deaths, 2088 cancer related deaths, 1620 infection related deaths, and 28 intentional self-harm related deaths are presented. Current antipsychotic exposure increased mortality rate with HR between 1.92 and 2.31 for cardiovascular, cancer, and infection related death. Cumulative antipsychotic dosages were most commonly associated with increased rates of mortality for cardiovascular and infection as cause of death, whereas the associations were less clear with cancer and intentional self-harm as cause of death.
ConclusionsWe showed that cumulative antipsychotic drug dosages increased mortality rates for cardiovascular and infection as cause of death. These findings highlight the need for further investigations of long-term effects of treatment and of possible sub-groups who could benefit from treatment.

A good understanding of the spent fuel matrix (UO2) behavior under predisposal activities conditions is required for the proper performance assessment of a final repository. Hence, the oxidation evolution of UO2 under dry interim storage conditions, as a main predisposal action within the Spanish strategy, needs to be addressed. For this aim, in this work a detailed in situ Raman spectroscopy study of the surface oxidation of a UO2.00 disk heated in the presence of synthetic air at 573 K is presented. The spectra analysis required two previous studies. In the first one, UO2+x powder samples with controlled degree of non-stoichiometry were identified by thermogravimetric analysis and subsequently characterized by Raman spectroscopy. The equations obtained from this study enable estimating the oxidation degree of any UO2+x sample (for x < 0.20) at atmospheric conditions. The second one was performed in order to use these equations for the in situ experiments (at 573 K), since the shift of the bands due to temperature needs to be taken into account. Thus, the behavior of the Raman spectra as a function of temperature was analyzed and a correction term thereafter introduced in the initial equations.

High-fidelity simulators (HFSs) have been shown to prompt critical actions at a level equal to that of trained human actors (HAs) and increase perceived realism in intrahospital mass-casualty incident (MCI) exercises. For unannounced prehospital MCI exercises, however, no data are available about the feasibility of incorporating HFSs. This case report describes the integration of HFSs in such an unannounced prehospital MCI drill with HAs and provides data about the differences concerning triage, treatment, and transport of HFSs and HAs with identical injury patterns. For this purpose, 75 actors and four high-fidelity simulators were subdivided into nine groups defined by a specific injury pattern. Four HFSs and six HAs comprised a group suffering from traumatic brain injury and blunt abdominal trauma. Triage results, times for transport, and number of diagnostic and therapeutic tasks were recorded. Means were compared by t test or one-way ANOVA.
Triage times and results did not differ between actors and simulators. The number of diagnostic (1.25, SD = 0.5 in simulators vs 3.5, SD = 1.05 in HAs; P = .010) and therapeutic tasks (2.0, SD = 1.6 in simulators vs 4.8, SD = 0.4 in HAs; P = .019) were significantly lower in simulators. Due to difficulties in treating and evacuating the casualties from the site of the accident in a timely manner, all simulators died. Possible causal factors and strategies are discussed, with the aim of increasing the utility of simulators in emergency medicine training.

Evaluating the clinical and economic burden of healthcare-associated infections during hospitalization for surgery in France

Over 4 million patients suffer nosocomial infections annually in the European Union. This study aimed to estimate the healthcare burden associated with healthcare-associated infections (HAIs) following surgery in France, and explore the potential impact of infection control strategies and interventions on the clinical and economic burden of disease. Data on the frequency of HAIs were gathered from the 2010 Programme de Médicalisation des Systèmes d'Information (PMSI), and cost data were taken from the 2009 Echelle Nationale de Coûts à Méthodologie Commune (ENCC). It was estimated that 3% of surgical procedures performed in 2010 in France resulted in infection, resulting in an annual cost of €57 892 715. Patients experiencing a HAI had a significantly increased mortality risk (4·15-fold) and an increased length of hospital stay (threefold). Scenario analysis in which HAI incidence following surgery was reduced by 8% (based on a study of the effectiveness of triclosan-coated sutures), suggested that, annually, 20 205 hospital days and €4 588 519 could be saved. Analyses of 20% and 30% reductions in incidence (based on an estimate of the number of preventable nosocomial infections) suggested that annual savings of €11 548 057 and €17 334 696, respectively, could be made. New infection control interventions which reduce HAI incidence during hospitalization for surgery have the potential to provide valuable cost savings to healthcare providers.

Cropping systems that rely on renewable energy and resources and are based on ecological principles could be more stable and productive into the future than current monoculture systems with serious unintended environmental consequences such as soil erosion and water pollution. In nonagricultural systems, communities with higher species diversity have higher productivity and provide other ecosystem services. However, communities of well-adapted crop species selected for biomass production may respond differently to increasing diversity. Diversity effects may be due to complementarity among species (complementary resource use and facilitative interactions) or positive selection effects (e.g., species with higher productivity dominate the mixture), and these effects may change over time or across environments. Our goal was to identify the ecological mechanisms causing diversity effects in a biodiversity experiment using agriculturally relevant species, and evaluate the implications for the design of sustainable cropping systems. We seeded seven perennial forage species in a replicated field experiment at two locations in Iowa, USA, and evaluated biomass productivity of monocultures and two- to six-species mixtures over 3 years after the establishment year under management systems of contrasting intensity: one or three harvests per year. Productivity increased with seeded species richness in all environments, and the positive relationship did not change over time. Polyculture overyielding was due to complementarity among species in the community rather than to selection effects of individual species. Complementarity increased as a log-linear function of species richness in all environments, and this trend was consistent across years. Legume–grass facilitation may explain much of this complementarity effect. Although individual species with high biomass production had a major effect on productivity of mixtures, the species producing the highest biomass in monoculture changed over the years in most environments. Furthermore, transgressive overyielding was observed and was more prevalent in later years, in some environments. We conclude that choosing a single well-adapted species for maximizing productivity may not be the best alternative over the long term and that high levels of species diversity should be included in the design of productive and ecologically sound agricultural systems.

Objective:
Management of patients with head and neck squamous cell carcinoma is often based on clinical parameters, with little appreciation of the underlying tumour biology. Single biological marker studies fail to acknowledge the complexity of these tumours. Our aim was to define a profile of biological markers associated with outcome.
Design:This retrospective study involved consecutive patients with oropharyngeal squamous cell carcinoma treated with primary radiotherapy between 1996 and 2001. Pre-treatment biopsies were used to study the immunohistochemical expression of nine biological markers. Markers were chosen to reflect biologically relevant pathways.
Results:Following analysis of nine markers, a profile of two markers was derived (carbonic anhydrase 9 and major vault protein), the co-expression of which conferred a significantly poor probability of locoregional control. The prognostic effect of these biomarkers in combination was greater than their effect individually.
Conclusion:Biomarker profiles can be established which highlight large differences in locoregional control. Identifying tumours that express both carbonic anhydrase 9 and major vault protein may facilitate patient selection for more aggressive treatment.

Commission 12 covers research on the internal structure and dynamics of the Sun, the “quiet” solar atmosphere, solar radiation and its variability, and the nature of relatively stable magnetic structures like sunspots, faculae and the magnetic network. There is considerable productive overlap with the other Commissions of Division II as investigations move progressively toward the fertile intellectual boundaries between traditional research disciplines. In large part, the solar magnetic field provides the linkage that connects these diverse themes. The same magnetic field that produces the more subtle variations of solar structure and radiative output over the 11 yr activity cycle is also implicated in rapid and often violent phenomena such as flares, coronal mass ejections, prominence eruptions, and episodes of sporadic magnetic reconnection.
The last three years have again brought significant progress in nearly all the research endeavors touched upon by the interests of Commission 12. The underlying causes for this success remain the same: sustained advances in computing capabilities coupled with diverse observations with increasing levels of spatial, temporal and spectral resolution. It is all but impossible to deal with these many advances here in anything except a cursory and selective fashion. Thankfully, the Living Reviews in Solar Physicsacute; has published several extensive reviews over the last two years that deal explicitly with issues relevant to the purview of Commission 12. Cerebral palsy following term newborn encephalopathy: a population-based study

Cerebral palsy (CP) can occur in term infants with or without preceding newborn encephalopathy. We compared the type and severity of CP and associated disability in these two groups. Participants from a population-based case-control study of term newborn encephalopathy were followed up for 6 years and linked to the Western Australian Cerebral Palsy Register. The remaining term infants with CP for the same period were also identified from the Cerebral Palsy Register. 13% of neonatal survivors of term newborn encephalopathy had CP, a rate of 116 per 1000 term live births. Overall, 24% of term infants with CP followed newborn encephalopathy. CP following newborn encephalopathy was more likely to: affect males (72% vs 56%); be severe (47% vs 25%); and be of spastic quadriplegia or dyskinetic types. Cognitive impairment was more common (75% vs 43%) and severe (41% vs 16%), as was epilepsy (53% vs 29%) in survivors of encephalopathy. These children were also more likely to: be non-verbal (47% vs 22%); have a severe composite disability score (47% vs 26%); and die between time of diagnosis of CP and age 6 years (5-year cumulative mortality 19% vs 5%). Children born at term who develop CP following newborn encephalopathy have a poorer prognosis than those with CP who were not encephalopathic in the first week of life.
Far-Infrared Atomic Lines: PDRs or Shocks?

The relative role of the stellar radiation field, the stellar outflows and the interstellar radiation field (ISRF) in transforming the molecular ejecta into atomic gas was the subject of our ISO LWS and SWS spectroscopy study of 24 evolved stars which span the range from AGB stars to proto-planetary nebulae (PPNs) and PNs. The far-infrared (FIR) atomic fine-structure lines are powerful probes of the warm atomic gas in photodissociation regions (PDRs) and shocks. This paper summarizes and compares the ISO spectroscopy studies of carbon-rich (C-rich) and oxygen-rich (O-rich) evolved stars, published by Fong et al. (2001) and Castro-Carrizo et al. (2001), respectively. We find that photodissociation, not shocks, is responsible for the chemical change from molecular to atomic gas.
The effect of intrathecal midazolam on post-operative pain

Intrathecal midazolam for use as a post-operative analgesic when given alone and in conjunction with intrathecal diamorphine was assessed. Fifty-two patients scheduled for elective Caesarean section under spinal anaesthesia were randomly allocated to receive either bupivacaine (B), bupivacaine with diamorphine (BD), bupivacaine with midazolam (BM) or all three (BMD) by intrathecal injection. Post-operatively, no differences in visual analogue score (VAS), sedation or post-operative nausea and vomiting (PONV) could be demonstrated between groups. Patient-controlled analgesia system (PCAS) usage was significantly greater in group B when compared with the other groups. Pruritus was commoner in patients receiving diamorphine (BMD & BD). No side effects attributable to midazolam were identified. Intrathecal midazolam at this dose appears safe and has clinically detectable analgesic properties. The duration of useful analgesia appears to be short-lived.
